You need to learn about the RULES for each Court. 

Inside these rules are the samples of what you need to send to the other party, as well as many pitfalls and confusing language. READ CAREFULLY. Then take a break. Then READ AGAIN. Then ask on AVVO.com.

STATE Court Rules (CR) for your STATE

SUPERIOR COURT RULES for your COUNTY

Then find out which COURT you will be in and research 
LOCAL COURT RULES (LCR's) for the ACTUAL court

THEN, search for Judge's Orders. These are the last details that amended or waive procedures and will save you time. You need to know the Judge of your case.

The Local Court Rules are a good starting point, and you can work backwards through the rule sets to fill in any blanks.
